Request an Appointment
You can request an appointment for an assessment or consultation by completing the Success in Mind online Contact Form. So that we can respond to your request as quickly as possible, please note:
The information collected is confidential and necessary for us to determine whether our services will meet the needs of your child. It will take several minutes to complete.
We will ask you questions about:
- Specific concerns related to your child’s learning
- Your estimate of his/her academic skill levels
- Previous assessment findings
- Current services and supports
If you are unable to complete our online Contact Form, you may print it and fax or mail it to us.
Receive confirmation
After you submit the online Contact Form, the following will happen:
- You will receive an automated email response from Success in Mind confirming that we received your Contact Form. To ensure delivery of these important emails, you will want to add success-in-mind.org to your email safe list or address book.
- Our staff will review your Contact Form to ensure that our services are a match for your child’s needs and your concerns. We may contact you by phone with additional questions.
- You will be contacted by phone, email, or regular mail regarding your request for an appointment, typically within three days of submitting the Contact Form. If you have a preference for how and when we should contact you, please provide that information in the Parent Information section on the Contact Form.
Appointment Preparation
Once your assessment has been scheduled, you will receive information about next steps, including completing online questionnaires and homework packets and ideas for talking to your child about what to expect at the assessment.
